SuperKidsSummerCamp – Waiver and Release of Liability
Location: 2601 E State Highway #4, Lewisville, TX 75056 Camp Age Group: 6 – 16 years Activities: Educational, Recreational, Sports, Fun, and Entertainment Waiver and Release of Liability By checking the box below, I, the undersigned parent or legal guardian of the camper listed above, acknowledge that I have read and agree to the terms and conditions outlined below for participation in SuperKidsSummerCamp:
1. Assumption of Risk I understand that participation in camp activities may involve physical activity and carries the risk of injury, illness, or loss. I accept that all activities—including educational, recreational, and sports-based—are voluntarily undertaken at the participant’s own risk.
2. Release of Liability I release and hold harmless SuperKidsSummerCamp, its staff, volunteers, affiliates, and facility owners from any claims or liabilities for injuries, damages, or losses incurred during camp participation.
3. Medical Treatment and Expenses In case of emergency, I authorize staff to obtain necessary medical treatment for my child and accept responsibility for any related expenses.
4. Personal Property I understand that the camp is not liable for any lost, stolen, or damaged personal belongings.
5. Behavior and Dismissal I agree that if my child exhibits harmful or disruptive behavior, the camp may dismiss them from the program without refund.
